    • Medical

      FXI offers three Independence Blue Cross plans from which to choose: the Standard PPO Plan, the Consumer High Deductible Health Plan and the Qualified High Deductible Health Plan. Preventive services are covered 100% (No Co-pay/No Deductible) under all programs. If you elect the Qualified High Deductible Health Plan, FXI will make a contribution to your Health Savings Account.

      Prescription drugs are covered under the medical program through CVS / Caremark. Prescription drugs can be filled through retail pharmacy or mail order. Maintenance drugs require a three month supply which can be filled either through mail order or any retail CVS pharmacy for only two copays.


      FXI offers dental coverage through MetLife. The plan covers a variety of procedures, including 100% coverage for preventive care with an annual $1,700 limit. The plan also includes an orthodontic benefit for dependent children.


      FXI offers vision care coverage through Vision Service Plan (VSP), VSP covers eye exams, frames, lenses and contact lenses.

      Life and AD&D

      FXI provides Basic Life and Accidental Death and Dismemberment (AD&D) coverage. You may purchase additional life and AD&D insurance at group rates, as well as life and AD&D insurance for dependents.

      Travel Accident Insurance

      FXI provides business travel accident insurance for certain employees who travel on Company approved business. The coverage is equal to 5 times your salary.

      Disability Insurance

      FXI provides short-term disability for a non-work related illness or injury. Salaried employees can purchase long-term disability coverage to provide income continuation beyond the first six months of illness or injury.

      Flexible Spending Accounts

      FXI allows you to place pre-tax dollars in a medical reimbursement account for non-covered medical expenses or in a dependent care reimbursement account for eligible childcare or eldercare expenses.

      Educational Assistance

      FXI offers tuition reimbursement for pre-approved courses that are in a degree program and are related to your job. Under this program you must complete the course with a grade of “C” or better. The plan provides reimbursement for up to $5,250 in a calendar year.


      FXI provides paid vacations and holidays for eligible employees.

      401(k) Savings Plan

      The FXI 401(k) Savings Plan offers a generous company match. You can contribute a portion of your pay on a pre-tax basis (subject to IRS limits) through payroll deductions. FXI makes a matching contribution of 100% on the first 3% of your deferred pay and 50% on the next 2% of deferred pay. This match is immediately vested.
